Dementia Support

We are one of the few hospices in the UK that support those living with Dementia
This service provides specialist palliative care and dementia management advice to those caring for individuals who are experiencing difficulties in the later stages of Dementia.

The Hospice understands that people with Dementia suffer disadvantage in accessing health care services, contributing to them experiencing less than optimal care during the end of life. Our team offers support and education to both formal and informal carers, we advise on both medical and non-medical approaches to symptoms that may arise.
Patients are holistically assessed in their current settings, we aim to support our patients to remain in their preferred setting, as we know a change of environment can be detrimental to them. We aim to monitor patients regularly in order to be responsive to any adverse changes they may encounter.
We are aware of the need to support and care for carer’s, with this in mind we have developed Carer support groups that run throughout the year. This can be accessed at any point in the Dementia journey. Carer’s can attend the support group knowing their loved one is looked after by our experienced staff and volunteers.
Referrals can be made when individuals are experiencing adverse symptoms associated with their dementia, as they approach the later stages of Dementia. A formal diagnosis of dementia is not necessary, but significant cognitive impairment, as assessed by the referrer, and agreed by the specialist nurse is the criteria.
